About the role

The Clinical Lead Ready for Home will provide both clinical and managerial support across all aspects of the Ready for Home Out of Hospital Care program supporting the delivery of safe high-quality evidence-based care to older participants while also building staff capability and strengthening clinical governance.

About you

You will be an experienced AHPRA registered nurse possessing extensive clinical experience in aged care geriatrics or rehabilitation with a passion for providing clinical care to older participants along with demonstrated experience in the supervision of staff.

You will also demonstrate:

  • Proven ability to make sound clinical and operational decisions in complex situations
  • Strong knowledge of clinical governance quality improvement and risk management practices
  • Demonstrated cross-cultural experience and experience working with individuals from diverse backgrounds
  • Demonstrated ability to mentor supervise and build capability in staff teams
