Job Summary:

An opportunity to be part of an Innovative start up program. The first of its kind in King County. This position offers numerous opportunities for professional growth and development. Valley Cities provides outreach to underserved areas of King County. The goal is to assist individuals who may otherwise not have access to behavioral healthcare. The Mobile Care Navigator is an essential member of a team of mental health professional substance use disorder behavioral health primary care and other partnered providers who together will provide access to a continuum of services for underserved areas and population utilizing a harm-reduction philosophy of care. This position will work closely with a MHP and other Care Navigators to screen assess enroll and schedule clients for services and other needed care based upon individual care needs geography and timeliness of access.

Key Responsibilities:

  • SUD Assessment and Intakes
  • Assist with client screening as part of the mobile van program enrollment Medicaid enrollment scheduling of clinic visits procedures and follow up.
  • Provide medication education and support.
  • Provide individual Care Navigation and Case Management
  • Provide Information and referrals if needed outside the Agency.
  • Provide assistance with resources including referrals.
  • Family support and education
  • Collaborate with community partners to coordinate transitions of care between providers/agencies
  • Partner with Match team members in obtaining prescriptions medication orders and refills and communication as needed in keeping with scope of practice and applicable state and federal regulations.

Minimum Qualifications:

  • Associates degree.
  • SUDP required.
  • Ability to transport clients
  • Current Drivers License and automotive insurance.

Other:

  • Pass required background check.
