You will be updated with latest job alerts via emailThe Specialty Care & Community Partnerships Coordinator assists Care Teams in a multi-disciplinary clinic serving patients by coordinating all external (and internal specialty) medical visits for client panels. This position will work with our Downtown and East Clinics.
Work as part of a cohesive and innovative medical referrals team to provide support for medical Care Teams and connect under-served patients with needed medical referrals - both internal/external for all clinic and outreach clients including:
Verifying the clients insurance Project Access or other financial assistance programs
Locating appropriate specialty clinic offices that will accept the client
Sending all required chart information and other paperwork required to specialty clinic
Completing all prior authorizations as required by insurance
Scheduling specialty appointments as needed
Scheduling interpretation for the appointment as needed
Communicating the appointment and scheduling details to clients including what to bring special instructions and expectations
Following up with external office records as needed
Follow-up with the care team regarding details of the appointments and follow-up (i.e. barriers transportation medical housing appointment reminders etc) as needed
Advocate for patient with external organizations and clinics as they navigate the referral process
Responsible for developing expertise in navigating insurance systems and will work with the front desk to identify and understand how insurance impacts the referral process
Build and maintain relationships with community social service partners
Attend staff meetings and clinic trainings; assist in training staff and/or volunteers in the referrals processes; attend provider meetings and other staff meetings as needed
Identify opportunities for improvement to clinic processes and workflows; participate in process improvement projects
Sorting prioritizing and processing referrals
Navigating the insurance system and completing all necessary prior authorizations
Following up after completion of referrals to request medical records from specialty clinics
Coordinating and managing in house specialty clinics and coordination with outside partnerships such as the Casey Eye Institute Medical Teams International Pacific University
Identify and address difficulties and barriers within referral process.

Strong reception and administrative skills with high accuracy and attention to detail
Education and Experience
At least one year related experience in a clinical setting
Must be able to pass Department of Health Services criminal background check
Bilingual fluency preferred
Medical training or experience preferred
Working Conditions
This job is located in a standard medical office environment.
Physical Requirements
This job requires operating phones computers and other office equipment. Communicating is required on a regular basis. Moving inside the building to other offices and program delivery spaces as well as moving to surrounding areas is expected. Sporadically moves boxes weighing up to 20 pounds.
