Our Maternal Fetal Medicine team manages diabetes in pregnant patients. The team consists of APRNs Registered Nurses and Registered Dieticians. Workload is shared between 2 Diabetes Educators and supported by MFM Providers and Dietitians. Leading group and individual classes throughout the week is balanced with messages and phone calls with patients.Summary
Educates patients on self care involved in the diagnosis of diabetes; collects patient data to monitor health status and proactively manage the patients condition.Job Description
Organizes diabetes self care materials including nutrition medication physical activity and glucose testing. Educates patients on the necessary lifestyle changes involved in the diagnosis of diabetes. Demonstrates familiarity with pathophysiology epidemiology and clinical practice consistent with practice level. Collects patient data on a regular basis in order to monitor their health status. Provides nutrition therapy and monitors nutritional care of clients/patients of all ages including neonatal pediatric adolescent adult and geriatric. Ability to recognize evaluates and provide for nutritional needs based on cultural ethnic and religious beliefs. Makes suggestions on how to improve health conditions based on the analysis of patient data; refers patients with specific problems to other healthcare providers. Applies principles of program and/or business management to create a climate that supports successful self-management of diabetes. Educates patients on self-care and lifestyle changes involved in the diagnosis of diabetes.Qualifications
Minimum of a bachelors degree in dietetics or food and nutrition through an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited program.Sanford is an EEO/AA Employer M/F/Disability/Vet.
