Community Allied Health Clinical Coordinator

About the Role
Community healthcare provision is key to supporting our health system and community. Southern is going through exciting innovative changes to provide care to our community.
The community clinical coordinator role will support both inpatient and community teams to coordinate efficient effective and safe services within available resources. This role is varied with its main driver being patient flow it will coordinate and work with teams to get people home living quality lives.
The initial purpose will be to embed clear systems to ensure patient flow across the hospital and community for ACC contracts. Coordination of interim care into ARC facilities and relationship building with interim care facilities will be key in this role. You will work alongside community teams inpatient teams ARC facilities primary care agencies unit managers and other clinical coordinators. The role has the potential to grow and change with our change in modes of care between the hospital and community the opportunities are endless
This role provides leadership for the coordination of clinical service delivery clinical leadership and direction to staff across community.

This position does not encompass budget responsibility but has delegation of authority for management of staff within the community team.
