Description of Work



This position is 100% federally funded.

**Position is Exempt from the State Human Resources Act - G.S. 126**

**Position is eligible for a hybrid-teleworking schedule to include weekly onsite work hours**

**POSITION IS TIME-LIMITED through December 2031**
**PENDING RECLASSIFICATION**

The primary purpose of this position is to monitor and enhance the quality and effectiveness of transition services provided to individuals with Intellectual/Developmental Disabilities (IDD) and Traumatic Brain Injury (TBI) through the Money Follows the Person (MFP) program. The role involves providing technical assistance analyzing program data and ensuring accurate and timely reporting based on documentation submitted by transition coordination vendors.

The MFP Transition Services Monitor IDD will support program improvement by collecting reviewing and synthesizing data trends to inform decision-making and improve service delivery. This position reports directly to the MFP Project Director and plays a key role in promoting successful community transitions for individuals leaving institutional settings.

Masters degree in a discipline relative to the program from an appropriately accredited institution and two years of related experience;

OR

Bachelors degree in a discipline relative to the program area from an appropriately accredited institution and four years of related experience;

OR

an equivalent combination of education and experience.
