Job Title: Nurse Practitioner - Wound Care

Job Responsibilities:
- Conduct thorough assessments of ostomy patients evaluating wound status skin integrity and overall health.
- Develop and implement individualized care plans to ensure optimal wound healing and management of complications.
- Provide education on ostomy care techniques skin maintenance and prevention of complications.
- Collaborate with physicians nurses and healthcare professionals to coordinate and ensure continuity of care.
- Utilize evidence-based practices and advanced wound care techniques to promote healing and prevent infection.
- Maintain accurate documentation of patient assessments interventions and outcomes in compliance with regulatory standards.
- Work closely with surgical teams for pre-surgery site markings and patient education regarding ostomy procedures.
Qualifications:
- Board Certified Nurse Practitioner.
- New York State License (or willing to obtain).
- Wound Care Certification (WOCN) is required.
- Knowledge and experience in wound care and ostomies.
- DEA License (or willing to obtain).
- At least 1 year of NP employment experience preferably in wound care management with ostomy care (highly preferred).
License and Certification Requirements:
- Board Certification (required by start date).
- New York State License (required).
- Wound Care Certification (WOCN) (mandatory).
- DEA License (required).
