Job Role:Supported Housing Worker

Salary: 25235 per year

Hours: 37.5 hours per week

Contract type: Permanent

Location: Trowbridge Wiltshire

About the Role:

As a Supported Housing Worker within the Connect Wiltshire service youll provide person-centred high-quality support and housing management to single homeless clients with low to medium support needs living in designated properties. Your focus will be on helping individuals progress towards greater independence ensuring sustainable move-on within defined timescales.

Connect Wiltshire is a multi-agency partnership led by Turning Point and commissioned by Public this role youll play a pivotal part in delivering the supported accommodation element of the service. Youll work closely with partner agencies to support clients with complex needs many of whom are in early recovery or abstaining from drugs and alcohol.

Responsibilities include:

• Manage referrals into the service ensuring a smooth and supportive assessment process.
• Create and deliver personalised support plans and risk assessmentsto help clients thrive.
• Champion safeguardingby following policies and procedures to keep everyone safe.
• Empower clientsby offering advice information and support to connect them with the right services.
• Oversee the day-to-day runningof the service making sure everythings safe running smoothly and in line with health and safety standards.

Qualifications / Requirements:

• Experience working with people who may have complex needs and/or challenging behaviours.
• Able to build effective relationships with clients and services.
• A valid UK driving licence and access to own vehicle is required - business insurance will be required for roles involving travel to work
