Job Description
To assist the patient and his/her family or members of the medical health care team in meeting post-hospital health needs through appropriate teaching psychosocial evaluation and support and referral to community agencies that can provide procedures and treatment measures to restore the patient to his/her optimal physical and mental health.
Minimum Education:Master of Social Work or LCSW Preferred.
Minimum Experience/Knowledge:
Working knowledge of local regional and federal health social and welfare resources
Required License/Certification:Current BLS certification from the American Heart Association.

The salary range for this position is $68640 - $110000. When extending an offer of employment the University of Southern California considers factors such as (but not limited to) the scope and responsibilities of the position the candidates work experience education/training key skills internal peer equity federal state and local laws contractual stipulations grant funding as well as external market and organizational considerations.
