With over 400 employees in Southern Colorado you would be joining a mighty team of support and administrative staff case managers clinicians physicians nurses and others in our efforts to improve the health and wellbeing of our community.

Were hiring a Clinical Care Specialist to join our Community Access Recovery and Engagement (CARE) team at 41 Montebello Road in Pueblo.

What Youd Be Doing

As the Clinical Care Specialist you would play a central role in helping our clients access services and take the next steps to wellness. Your work would take you to homes and other settings and call upon you to collaborate with other professionals within and outside our agency to assure that clients receive the very best care available.

Specifically the Clinical Care Specialist

  • Maintains a caseload and conducts case management services for clinicians;
  • Makes appropriate referrals to address mental health and substance use needs;
  • Links clients and community resources effectively;
  • Offers behavioral health counseling and outreach;
  • Ensures clients attend scheduled appointments in the community and at Health Solutions;
  • Identifies stressors and precursors that led to the current situation or crises;
  • Identifies available coping mechanisms and natural supports;
  • Enhances problem-solving independent living and socialization skills among clients;
  • Provides support for clients to remain in home and community to avoid more restrictive living arrangements;
  • Maintains accurate records in the agencys electronic health record; and
  • Performs other job-related duties assigned by the director or designee.
  • Physical requirements include the following:
  • Must be ableto remain in a stationary position at least 50% of the time.
  • Frequently communicates with physicians about medical notes must be able to exchange accurate information in these situations.
  • Constantly operates a computer and other office productivity machinery such as a calculator copy machine and computer printer.
  • Occasionally lifts and carries items up to 20 pounds
  • Occasional exposure to communicable diseases and bodily fluids

Qualifications

  • Bachelors degree in social work psychology or a closely related field
  • Computer proficiency
  • Colorado drivers license a good driving record and access to transportation during work hours
