Job Description

The UI Health Care weekend option clinical pharmacist is responsible for ensuring that patients are safely and effectively transitioned from the hospital to their home or another care setting at both the North Liberty and Main Campus locations. This role involves reviewing medication regimens educating patients about their prescriptions providing counseling and ensuring proper medication reconciliation to prevent errors after discharge. The clinical pharmacist will work three eight-hour shifts as outlined below. This schedule could be subject to change.

Saturday: 12:30 2100 at Discharge Pharmacy (Main Campus)

Sunday: 12:30 2100 at Discharge Pharmacy (Main Campus)

Position Responsibilities:

  1. Clinical Care: Accurately and Efficiently Dispense Prescribed Medications
    • Provide Therapeutic Drug Monitoring (TDM) on take-home prescription orders through evaluation of the patients medications and medical history
    • Evaluate all prescription orders to ensure the appropriateness of the following:
      • Indication for use
      • Dose
      • Route of administration
      • Drug Interactions
      • Dose adjustments for organ dysfunction
      • Physical and therapeutic incompatibilities
      • Drug-induced side effects and adverse reactions
      • Potential drug misuse or abuse
      • Use of the most cost-effective therapy
      • Duplicate therapy
  2. Education: Provide Medication Counseling
    • Counsel patients on the proper use potential side effects interactions and storage of prescribed medication
    • Answer patient questions related to their medications health conditions and general wellness
    • Evaluate patient compliance with prescribed medication therapy and identify problems that may lead to partial or noncompliance
  3. Operational: Review and Interpret Prescriptions to Ensure Patient Safety
    • Utilize hospital-wide programs intended to improve care (e.g. the Drug Use Evaluation Program and Adverse Drug Reaction Reporting Program
    • Follow-up on any medication system problems such as:
      • Unclear or inappropriate prescription orders
      • Cost effective drug therapy
      • Prescription for non-formulary products
  4. Collaboration: Collaborate with Healthcare Providers to Provide Optimal Patient Care
      • Communicate with prescribers about patient medication needs
      • Provide appropriate drug information to other healthcare professionals
      • Utilize appropriate resources and coordinate the resolution of financial concerns/burdens with department financial counselors
  5. Documentation: Document All Patient Care Activities Performed
      • Operate within the patients medical record and other online reporting systems to document the following activities when applicable:
        • Identification and documentation of patient drug allergies
        • Medication counseling provided to patients
        • Interventions made with prescribers and other healthcare providers
        • Participation in Drug Use Evaluation Program activities
  6. Precepting: Mentoring Pharmacy Learners
        • Provide direct supervision and mentorship to pharmacy residents students and interns
        • Educate learners on proper medication management at discharge including counseling medication reconciliation and follow-up care
        • Deliver real-time feedback assess performance and provide constructive guidance for improvement

Weekend Option Hours of Work and Scheduling:

Staff members are scheduled to work 24 hours between 3:00 p.m. Friday and 7:30 a.m. Monday. A 50% weekend pay differential will be applied to all hours worked within the defined weekend timeframe. If any of these hours qualify for shift differential it will be paid in addition to base pay; but no additional weekend differential will be paid. Staff will be scheduled to work 24 weekends within a six-month period and can take up to 48 hours of vacation (if accrued) in that timeframe. Staff with a seniority level of 6 years can be scheduled an additional 24 hours off per year.
