This position provides care coordination to psychiatric hospitals the justice system and the Office of Civil and Forensic Mental Health (OCFMH) Emergency Medical Services Facilities (EMS Facilities) when they are working with individuals on emergency mental health holds that may expire prior to finding appropriate placement for evaluation and treatment. As a Social Work/Counselor III this position provides care coordination primarily for individuals under Article 65 of C.R.S.Care Coordination for persons certified or in need of ongoing treatment. (1) A facility designated by the commissioner shall notify and engage the BHA prior to terminating or transferring a person certified pursuant to sectionsor. The BHA may provide care coordination services to support a person whose certification is terminated but who is in need of ongoing treatment services. (2) The BHA shall directly or through contract provide care coordination services to a person certified pursuant to sectionor and determined by the designated facility and the BHA to need care coordination services.
Care coordination for the identified priority populations served by the BHA is also part of this position. Collaboration across the BHA CDHS Office of Civil Forensic Mental Health Behavioral Health Administration Service Organizations and between state agencies such as Health Care Policy and Finance is necessary to ensure individuals needing ongoing care coordinated through multidisciplinary processes involving multiple systems. This position may also be involved in the Emergency and Involuntary Commitment for Substance Use treatment processes under sectionC.R.S. andC.R.S. respectively.
Duties of the position:This position works closely with psychiatric hospitals the justice system and the Office of Civil and Forensic Mental Health (OCFMH) when they are working with individuals on emergency mental health holds that may expire prior to finding an appropriate placement for evaluation and treatment.
Collaborates with the program manager for transitional support to ensure there is continuity of care and coordination. This position provides care coordination for individuals under 27-65 under involuntary mental health treatment and priority populations identified by the Behavioral Health Administration. The position requires knowledge of established theories principles conceptual models and professional standards of care coordination. This position collaborates with other staff in BHA Programs to assist with the priority populations that require additional training and support considerations.
This position interacts regularly with stakeholders who are interested in services and service outcomes for the population defined as this positions area of expertise. Strong customer service skills are needed to assist all stakeholders.
Cross Divisional CoordinationThis individual may be collecting and organizing metrics from identified sources or they are responsible for the tasks performed by others who have been delegated the duties.
General inquiry processes are within the scope of this position and the Director of Care Coordination. Cross-division coordination is necessary based on the entry point of the inquiry question concern or complaint or possible facilitation of care coordination. It may be necessary to collaborate with other functional work units teams or divisions to address the stakeholder.
Education/Licensure/Certification/Experience:
A bachelors or masters degree in social work counseling or a related behavioral health field from an accredited college or university is required.
Current valid Colorado licensure as a:
Licensed Social Worker (LSW) OR
Licensed Clinical Social Worker (LCSW) OR
Licensed Professional Counselor (LPC) OR
Licensed Marriage and Family Therapist (LMFT)
Licensed Addiction Counselor (LAC)
And one (1) year of post-licensure professional experience in behavioral healthcare specifically in care coordination case management or a related clinical capacity. This experience must include working with individuals on emergency mental health holds and certifications under CRS 27-65as well as an understanding of the Colorado behavioral health system.
