Position Summary
We are seeking a caring and knowledgeable PRN In-Home Hospice Dietitian to join our interdisciplinary team. This role provides nutrition assessment counseling and support for hospice patients in their homes ensuring comfort and individualized care during end-of-life. This position is perfect for someone looking for a flexible schedule and looking to better serve the members of our community!
Responsibilities
Conduct in-home nutrition assessments for hospice patients as needed.
Develop personalized care plans that align with patient goals medical conditions and comfort measures.
Collaborate with the hospice care team including nurses social workers and physicians.
Provide education and guidance to patients families and caregivers on nutrition-related concerns in the home setting.
Maintain timely and accurate documentation according to agency policies and regulatory standards.
Qualifications
Registered Dietitian Nutritionist (RDN) credential required.
Current South Dakota licensure (or eligibility to obtain).
Hospice or palliative care experience preferred but not required.
Strong communication skills and a compassionate patient-focused approach.
Ability to work independently in patients homes and as part of a multidisciplinary team.
Schedule & Compensation
PRN (as needed) flexible scheduling based on patient needs.
Competitive hourly rate based on experience.
