Job Description

RN Case Manager (Permanent Role)

Location: Gainesville FL 32601
Shift: 10-hr Variable Shift
Guaranteed Hours: (40 hours/week)
Compensation: (Negotiable based on experience)

Key Responsibilities:
  • Coordinate and monitor the assessment treatment and discharge planning of patients.
  • Perform comprehensive case assessments and develop individualized care plans.
  • Collaborate with physicians social workers nursing staff and other health professionals to ensure optimal patient outcomes.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Educate patients and families on treatment plans and post-discharge care.
  • Ensure compliance with all hospital policies state regulations and Joint Commission standards.
  • Maintain accurate documentation of case management activities in the medical record.
  • Act as a liaison between the hospital and external agencies to support continuity of care.
Qualifications:
  • Licensure:
    • Active Florida RN license or multistate/compact RN license.
    • Compact license holders must obtain a Florida license within 60 days of relocation.
  • Certifications:
    • BLS (Basic Life Support) American Heart Association Required within 30 days of start.
    • Certification in Case Management Utilization Review or Nursing Preferred.
  • Experience:
    • Minimum 3 years of RN experience in an acute care setting required.
    • Recent acute care Case Manager experience (within the last year) is strongly preferred.
    • Will consider candidates with:
      • 2 3 years of ICU or strong Med-Surg experience or
      • Case management experience in a LTAC (Long-Term Acute Care) facility.
  • Education:
    • Graduate of an accredited school of nursing as an RN.
    • BSN preferred.
  • Skills:
    • Strong critical thinking and problem-solving skills.
    • Ability to assess patient needs and develop appropriate care plans.
    • Excellent communication coordination and organizational abilities.
