RN Case Manager (Permanent Role) Location: Gainesville FL 32601
Shift: 10-hr Variable Shift
Guaranteed Hours: (40 hours/week)
Compensation: (Negotiable based on experience)
Key Responsibilities: - Coordinate and monitor the assessment treatment and discharge planning of patients.
- Perform comprehensive case assessments and develop individualized care plans.
- Collaborate with physicians social workers nursing staff and other health professionals to ensure optimal patient outcomes.
- Monitor patient progress and make adjustments to care plans as needed.
- Educate patients and families on treatment plans and post-discharge care.
- Ensure compliance with all hospital policies state regulations and Joint Commission standards.
- Maintain accurate documentation of case management activities in the medical record.
- Act as a liaison between the hospital and external agencies to support continuity of care.
Qualifications: - Licensure:
- Active Florida RN license or multistate/compact RN license.
- Compact license holders must obtain a Florida license within 60 days of relocation.
- Certifications:
- BLS (Basic Life Support) American Heart Association Required within 30 days of start.
- Certification in Case Management Utilization Review or Nursing Preferred.
- Experience:
- Minimum 3 years of RN experience in an acute care setting required.
- Recent acute care Case Manager experience (within the last year) is strongly preferred.
- Will consider candidates with:
- 2 3 years of ICU or strong Med-Surg experience or
- Case management experience in a LTAC (Long-Term Acute Care) facility.
- Education:
- Graduate of an accredited school of nursing as an RN.
- BSN preferred.
- Skills:
- Strong critical thinking and problem-solving skills.
- Ability to assess patient needs and develop appropriate care plans.
- Excellent communication coordination and organizational abilities.
