2 Month Contract with a local Authority
Job Purpose
- To support timely safe and person-centred hospital discharges through the completion of complex assessments for individuals on the Discharge to Recover and Assess (D2RA) Pathway 3. The role involves working collaboratively with health and community services to ensure appropriate care planning and support is provided with particular focus on mental capacity best interests decision-making and continuing health care (CHC) eligibility.
Key Duties/Accountabilities
- Undertake complex discharge planning assessments for individuals on D2RA Pathway 3 ensuring a strengths-based outcome-focused approach.
- Apply the Mental Capacity Act (MCA) in practice including assessments and best interests decision-making.
- Assess individuals for Continuing Health Care (CHC) funding working in partnership with health professionals and families.
- Ensure discharges are safe timely and meet the assessed needs of individuals promoting independence wherever possible.
- Work agilely across multiple hospital sites and from home maintaining effective communication with multidisciplinary teams.
- Maintain accurate and timely case records in accordance with legal professional and organisational requirements.
- Engage with service users and families in a respectful and empathetic manner supporting them through challenging transitions.
- Contribute to multi-agency meetings and care planning forums to ensure holistic and coordinated support for individuals.
- Work in compliance with Social Care Wales professional standards local safeguarding policies and data protection legislation.
Essential Experience Required.
- Experience in hospital discharge planning and knowledge of D2RA pathways especially Pathway 3
Essential Qualifications Required
- Qualified Social Worker with valid Social Care Wales (SCW) registration
- Car driver with access to own vehicle for travel between sites
