Overview

Clinical Pharmacist

St. Joseph Health Bryan/College Station Navasota Caldwell Madisonville TX. Since 1936 St. Joseph Health has been caring for the communities in and surrounding the Brazos Valley. We have a Level III Trauma Center the first Joint Commission certified Primary Stroke Center and the first accredited Chest Pain Center in the Brazos Valley. As an integrated healthcare system St. Joseph Health includes a comprehensive network of over 100 employed providers including primary care physicians specialists and advanced practice clinicians. The network includes more than 30 ambulatory clinics featuring primary care Express Clinics and imaging and diagnostic services.

Responsibilities

  • Responsibilities:
  • Demonstrates ability to document clinical activities and interventions such as consults patient teachings and other patient specific information.
  • Interprets medication orders and/or prescriptions for drug interactions and appropriate drug selection. Accurately enters / verifies the order or prescription into the computer system.
  • Verifies and checks medications prepared by other pharmacy personnel.
  • Resolves drug related problems by consulting with other health care professionals to achieve appropriate patient outcomes. Unclear orders written by a licensed provider are clarified with the licensed provider.
  • Achieves and maintains an adequate basic knowledge base to provide clinical services for all age groups served. Provides basic clinical services as required by policy and procedure.
  • Responsible for technician activities performance provides direction and guidance as needed.

Qualifications

Required Education & Experience:

  • Bachelor or Science in Pharmacy or Doctor of Pharmacy degree
  • One (1) year of pharmacist experience certification

Required Licensure:

  • Active State Pharmacist License in good standing in the state in which the employee works
  • Board Certification in a Pharmacy Specialty based on the specialty title under which the employee works
  • BLS-CPR
