What Youll Do:

• Provide in-home or on-site care at private residences senior living communities and long-term care settings

• Manage chronic conditions preventive care and mental health needs for an aging population

• Collaborate with interdisciplinary teams to deliver whole-person senior-focused care

• Build trusted relationships through empathy consistency and patient education

• Support care access and equity by removing barriers for vulnerable seniors

*Must have valid Drivers License and reliable transportation

The Role:

As a Senior Doc provider you will:

• Deliver in-home primary care and/or psychiatric care to seniors in private homes assisted living communities and skilled nursing facilities.

• Conduct thorough assessments and implement customized care plans to manage chronic conditions and mental health needs.

• Collaborate with interdisciplinary care teams to ensure whole-person care that meets the physical and emotional needs of our patients.

• Build trust through empathy education and proactive communication with patients and families.
