Role: Contract Full-Time: Until March 27 2026. 35 hours per week evenings and weekends may be required.
Closing date:September 5 2025
Salary: Starting salary range $31.01 - $33.22/hour. *This represents the first three steps in a seven-step salary grid. Candidate experience will be considered when determining the starting salary.

Location: The successful candidate will work primarily from our Alliston Office located at 35 Victoria Street East Unit 1 Alliston. Some travel to other CMHA YRSS locations will be required within York Region & South Simcoe.
Job Profile:
The Case Manager supports individuals aged 16 experiencing serious and persistent mental illness impacting daily functioning. Working collaboratively with clients the Case Manager helps identify strengths develop goals and connect to services and supports that promote recovery independence and improved quality of life. Services are individualized flexible and recovery-focused aligning with empowerment principles and the Intensive Case Management Standards (2012).

This position reports to and is supervised by the Manager People and Teams South Simcoe Case Management Program.
The Case Manager may be assigned to different programs as required based on need (i.e.: ACTT Case Management etc.)
Working Conditions: Office work community settings meetings in client homes and travel with clients.
