We are seeking interest from passionate team members for a 3-month fixed term acting Case Manager positionacross our Intensive Therapeutic Care programs.
About the Opportunity:
This is your chance to take on a meaningful short-term challenge and further develop your case management skills within a supportive therapeutic framework.
Youll be working closely with children and young people who may have disabilities or have experienced complex this role youll provide strength-based person-centred case management that empowers participants and supports their growth stability and connection to community.
Key Responsibilities:
Deliver high-quality case management to children and young people in our care
Coordinate care plans and support networks involving families stakeholders and multidisciplinary teams
Promote safety cultural sensitivity and therapeutic best practices
Ensure documentation and reporting meets compliance standards
What Youll Bring:
A degree in Social Work Psychology Nursing Social Welfare or related discipline
Experience working with children and young people from trauma backgrounds
Familiarity with relevant OOHC legislation and standards
Ability to work collaboratively in a fast-paced team environment
Cultural competency especially when working with Aboriginal and CALD communities
A valid drivers licence Working with Children Check
