Job Title: Health and Wellbeing Assessor
Location: Croydon
Division: Adult Care Services
Team: Adult Social Care LIFE Service
Contract: Permanent Full-Time (36 hours/week)
Salary: Grade 6 32925 to 33957

Were looking for a dedicated and knowledgeableHealth and Wellbeing Assessorto join ourLIFE (Living Independently For Everyone)service. This role is key to helping individuals regain independence and live fulfilling lives at home.
Youll work closely with a multidisciplinary team including Social Workers Occupational Therapists Physiotherapists and Intermediate Care Nurses supporting them by carrying out delegated assessments and interventions. Youll be based in the community hospital or peoples homes working independently across a variety of settings.
