JOB OVERVIEW:
Your Way Home Montgomery County operates as the countys coordinated response system to address housing crises for families and individuals experiencing or at risk of homelessness. Rooted in a housing first approach the program focuses on securing and maintaining permanent housing as the primary step toward stability. After housing is established Your Way Home connects participants with critical community health human and financial services to reduce the risk of future homelessness.
The Rapid Re-housing Program is a core component of Your Way Home providing housing-focused case management and financial assistance to households experiencing literal homelessness. This initiative aims to swiftly transition individuals and families into permanent housing while ensuring they receive the necessary supports to sustain long-term housing stability.
Program participants benefit from personalized case management both before and after moving into their new homes. Services include Tenant education Budgeting and household management Employment support and Crisis intervention. Additionally Your Way Home actively engages in landlord recruitment and retention fostering relationships with property owners and managers willing to provide housing opportunities for program participants. This collaborative effort strengthens housing access and promotes long-term solutions to homelessness in Montgomery County. This position is contingent upon the renewal of grant funding from the Montgomery County Department of Housing and Community Development.

The Housing Stability Coach provides direct support and case management services to Montgomery County households. The focus is on resolving housing crises and promoting long-term stability by connecting clients to subsidies services and community resources.
EDUCATION/EXPERIENCE REQUIREMENTS:
- Bachelors Degree in Social Work with a minimum of 2 years of experience OR
- Bachelors Degree in a related field with at least 3 years of experience in shelter transitional or supportive housing OR
- Masters Degree in Social Work or a related field.
- Clearances Required: Satisfactory Criminal History Child Abuse FBI Clearances Physical/TB Test and a valid drivers license (prior to employment).
ESSENTIAL JOB FUNCTIONS:
The following outlines key responsibilities. Additional duties may be assigned as needed:
- Assess client eligibility for housing programs through office and community-based interviews scheduled via the Your Way Home call center.
- Conduct comprehensive client assessments using tools designated by Your Way Home leadership.
- Identify housing needs and determine rental contributions developing consumer-driven Housing Stability Plans (HSP).
- Facilitate connections to supportive services based on assessment findings.
- Provide follow-up services to families and individuals in line with Your Way Home policies to monitor and guide progress on HSP goals.
- Maintain accurate client records in the Clarity Human Services (HMIS) database.
- Collaborate effectively with Your Way Home Program Managers Call Center staff Community Connection Navicates Shelter Providers and other Housing Resource Centers.

WORK ENVIRONMENT AND LOCATIONS:
- Primary Location: Norristown Neighborhood Center (Housing Resource Center).
- Work Distribution:
- 25% office-based
- 65% community-based (conducting assessments and follow-ups)
- 10% attending off-site trainings or meetings
- Occasional evening hours and rare weekend work may be required.
- Flexible scheduling is available.
PHYSICAL REQUIREMENTS:
- Ability to manage high-stress situations.
- Must have mobility to fulfill job responsibilities.
- Regular data entry requiring extended periods of keyboard use.
- Ability to sit for at least 2 hours at a time.
- Climbing stairs (second-floor office).
- Occasional reaching pushing and pulling.
