You will be updated with latest job alerts via emailThis position provides public health nurse home visits to families in the Family Connects Program who are receiving a home visit(s) shortly after this position you would typically provide two home visits in a day with time to prepare and document before and after these visits. You will provide multiple assessments to both birthing parent and baby to assess health needs resources and ways to support. Before the visit you will communicate with the client to confirm the appointment preview records and gather supplies needed. The activities done during a home visit include weighing and measuring the baby taking blood pressure of mom assessing postpartum depression support and resource needs assessing interpersonal violence conducting safety assessments reviewing needs for ongoing home visiting support making any referrals or plans for follow up visits etc. You will then document your visits in the electronic medical record for both mom and baby enter data into the preferred data system follow up on referrals supply needs and provide case management.
Regular meetings occur weekly for the Family Connects Team and monthly for the whole Maternal Child & Family teams. Most nurses are currently working four 10-hour work schedule Monday - Thursday. This position must have the CPR certification and will be required to complete training specific to the Family Connects program. Due to the responsibilities of this role this position is not eligible for remote work.
