Community Services Worker - Ambulatory Care

THE DEPARTMENT:Ambulatory Care isa system of primary care and specialty care medical clinics conveniently located throughout the County of clinicsprovide care on an outpatient basis at all dedicated team of healthcare professionals offer a range of services tailored to meet the specific needs of our community members experiencing homelessness- from basic medical care and preventative screenings to mental health support addiction services social services shelter and basic partnerships with local agencies we aim to provide medical treatment and support in navigating the complex challenge of homelessness. We foster a safe and welcoming environment where every patient feels valued respected and is treated with dignity and compassion.

THE POSITION: Under immediate supervision the Community Services Worker assists professional staff in providing transportation referral translation and program related services to culturally economically physically or mentally impaired or disadvantaged individuals in the community; and performs other duties as required.

THE IDEAL CANDIDATE:The ideal candidate for this is a compassionate community-oriented individual with experience or education in public service social sciences or a related field and a demonstrated commitment to serving diverse underserved and homeless populations. They are skilled in building rapport with individuals from a variety of cultural social and economic backgrounds and are adept at providing supportive services such as client transportation translation outreach and referrals to medical legal housing and employment resources. The candidate is knowledgeable about local community structures and resources understands the unique challenges faced by homeless individuals and is effective in addressing barriers to care by linking clients to both internal and community-based programs. They are organized dependable and capable of documenting services conducting basic health screenings and leading group presentations on health nutrition and life skills. Bilingual ability. A strong ability to work independently and collaboratively within a care team are highly desirable.

WHAT WE OFFER:
The County of Ventura offers an attractive compensation and benefits package. Aside from our base salary range an employee within this position may also be eligible for the following:

• Educational Incentive -An educational incentive of 2.5% for completion of an associates degree 3.5% for completion of a bachelors degree or 5% for completion of a graduates degree.
• Bilingual Incentive - Incumbents may be eligible for bilingual incentive depending upon operational need and certification of skill. Proficiency levels by exam are $0.69/hour (Level I) $1.00/hour (Level II) or $1.32/hour (Level III).
• Merit Increases -New employees are eligible for an initial 5% merit increase upon completion of at least 1040 hours (approximately 6 months) assuming work meets satisfactory standards. Subsequent merit increases will be upon completion of each additional 2080 hours (approximately one year) from the initial merit increase.
• Deferred Compensation -Eligible to participate in the Countys 401(K) Shared Savings Plan and/or the Section 457 Plan. If you participate in the County sponsored 401(K) plan the County will match a part of your contribution up to 3% of your salary on a per pay period basis. Following five (5) years of continuous County service an additional 2% non-elective contribution will be made to the 401(K) plan.
• Health Plans -you are afforded a flexible credit allowance for purchasing medical dental and/or vision insurance.
• Flexible Spending Accounts -Pre-tax benefit towards eligible medical dental and vision care expenses.
• Pension Plan -Participate in the Countys defined benefit pension plan that provides lifetime payments to retirees. Members are vested after attaining five years of retirement service credit. If eligible you may establish reciprocity with other public retirement systems such as CalPERS.
• Holidays -12 paid days per year which includes a scheduled floating holiday.

  TENTATIVE SCHEDULE:

Specific assignments vary according to the program assigned but may include all or a combination of the tasks described below:

• Answers inquiries and explains agency/program policies to individuals and groups within the community;
• Provides translation and/or services schedules appointments purchases food and supplies for clients who are physically or mentally impaired;
• Refers individuals and families to available community educational medical legal and employment resources; follows-up on referrals or appointments scheduled;
• Drives County vehicles to pick up or deliver clients or supplies; or may travel to various sites throughout the County to meet with community members or OneStop events within the County;
• Keeps records of clients served payments received and clinic activities; orders supplies and prepares routine reports;
• Conducts community outreach to provide information on health-related topics and services;
• Keeps up to date on health program guidelines and requirements and assists in enrolling patients in programs;
• Meets productivity and quality goals set by agency;
• Addresses barriers to care by linking patients to internal and community resources;
• Participates in care team huddles meetings and required training;
• May lead group presentations and workshops on specialized topics including substance abuse home management skills job skills or health and nutrition to help individuals and families develop a healthier or more self-sufficient lifestyle;
• May supervise clients during approved visitations or activities;
• Assists with case management of Health Care for the Homeless; inputting client data into Homeless Information Management System (HIMS); and
• Performs other related duties as required.

EDUCATION TRAINING and EXPERIENCE:

Any combination of education and experience which has led to the acquisition of the required knowledge and abilities. The required knowledge and abilities also can be obtained by:

One (1) year of public service experience in providing services to the general public or equivalent experience/education.

Note: Public services are those services provided to the general public by a public non-profit community service organization or a government entity.

Substitution: Fifteen (15) semester units of college course work in social sciences ethnic studies communications or a closely related field may be substituted for six months of experience up to a total of one year.

NECESSARY SPECIAL REQUIREMENTS:

• Must possess or obtain a valid California driver license by date of hire and maintain thereafter.
• Must be able and willing to travel to various sites throughout the County on a regular basis.

DESIRED:

• Bilingual (English/Spanish).
• Public service experience in the health care industry.
• Beginning skills in Word and Excel.
• Some experience working with homeless mentally ill or those with substance abuse issues.
