About the Health Care for Homeless Department
This position is part of the Health Care for homeless Program (HfH). The program has a base of services within St Vincents that currently support clients that are experiencing or at risk of homelessness. This service aims to build on integrating and better coordinating existing services within St Vincents hospital and the development of new services to meet the need of the target client group.
The Sister Francesca Healy Cottage sits within the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow through delivering integrated and coordinated care to clients across the hospital and community interface reducing duplication and fragmentation of services and ensuring client centred care planning. SVHM HIP consists of integrated hub sites across several locations including in Fitzroy Kew North Richmond and Northcote.
The Cottage is a five-bed health facility in a homely terrace house and targets clients with complex needs such as homelessness substance use mental health issues disability and family violence. The multidisciplinary team aims to provide physical social and emotional support within an environment of safe practice respect and dignity.
