Your role:
As the Lead Practitioner it is your role to liaise with DFFH monitor risk and safety plans and identify performance trends and areas for improvement.
The Future Pathways program is an innovative approach built on practice elements such as motivational interviewing communication skills building and goal setting to respond to identified needs and achieve real and demonstrable working collaboratively with children young people parents and carers they seek to address key intersectionality issues with families children and young people (e.g. substance use mental illness low income social isolation) as identified by each regional FPR Response.
This role supports programs in our South and Bayside regions and therefore will require travel between locations.
