The RN Cardiovascular Clinical Coordinator is a registered nurse who is accountable for effective
management coordination of care development of patient education data collection and application
and care management oversight for our cardiac patient population. Patients within this scope of care
include those who are being evaluated for advanced cardiovascular disease and cardiovascular
procedures and treatments. The Cardiovascular Clinical Coordinator guides inpatient and outpatient
care in partnership with the critical care service line interventional cardiologists the cardiothoracic
surgical team electrophysiologists advanced practitioners and clinical staff. Alongside this
multidisciplinary team the Cardiovascular Clinical Coordinator delivers evidence-based care with a
patient and family-centered approach and strives to meet or exceed the standards of quality care set
forth by AHMC Anaheim Regional Medical Center Critical Care Service Line.
The RN Cardiovascular Clinical Coordinator has a developed knowledge base and nursing expertise
in cardiovascular diseases and treatment modalities including invasive and surgical interventions
(TAVR CABG). Following consultation with multidisciplinary providers the RN Cardiovascular
Clinical Coordinator ensures continuity of care through communication with local and regional
referring physician providers and clinical staff. This position will report to the Director of Critical Care
Services Line.
The RN Cardiovascular Clinical Coordinator will:
Coordinate clinical management of inpatient cardiology patients: Facilitate care progression by
monitoring advancement towards care plan goals alerting the care team to potential care plan
revisions in response to changes in patient needs and condition advancing care plans to achieve
desired outcomes and maintaining open communication with individuals involved in patient care and
treatment. Problem solve with a multidisciplinary team to solve complex patient care issues. Prepare
and analyze pre-procedure clinical data ensuring verification and completion of physicians orders
and consult with the multidisciplinary team.
Participate in direct patient care with emphasis on patient education: Educate patients and
families on all aspects of disease management diagnostic procedures and interventional

California state RN licensure
Bachelors degree Nursing (required)
Masters of Science Nursing degree (preferred)
ACLS certification through American Heart Association
BLS certification through American Heart Association
