The Medical Mgmt Social Worker 2 provides supportive counseling to patients and families assesses patient discharge needs and coordinates discharge services. Relies on education experience professional training and judgment to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision.
TASKS
continuous documentation of productivity for the department.
data for the purpose of Continuous Quality Improvement.

in patients charts with accuracy and timeliness on behalf of the patient as necessary
on call service on a rotating basis with other members of Social Service staff.
and participates with other staff members by sharing referrals taking extra call answering telephones order to meet patients needs in the absence of another staff member
