Location: Massachusetts (statewide travel required)

Fortcom Health Services is currently seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Adult Family Care (AFC) and Adult Foster Care program. This full-time position involves monthly in-home visits to members and comprehensive case management to ensure the continuity and quality of care.

About the Role:

The LPN will manage a caseload of approximately 6090 members. In addition to conducting required monthly home visits the LPN is responsible for maintaining all related documentation coordinating care needs and ensuring each member meets the criteria for continued program eligibility. This includes clinical assessments care planning caregiver training and collaboration with the interdisciplinary team and healthcare providers.

Primary Responsibilities:

  • Complete the MDS-HC and other required clinical assessments.
  • Develop implement and update AFC plans of care on an ongoing basis.
  • Train evaluate and supervise AFC caregivers in collaboration with the Program Director.
  • Monitor members health status and report any significant changes to their physician.
  • Review physician summaries and ensure care is aligned with medical instructions.
  • Conduct admission visits and complete corresponding nursing progress notes.
  • Periodically review caregiver logs for accuracy and completeness.
  • Submit semiannual health status reports to each members primary care provider.
  • Plan for and manage member discharges from the AFC program as needed.
  • Conduct orientation for caregivers before they begin providing personal care.
  • Provide ongoing training and support to caregivers on topics such as health and aging.
  • Maintain up-to-date and compliant documentation for each member in your caseload.
  • Perform additional office or program duties as assigned by the Program Director or COO.
Qualifications:

  • Active LPN license in good standing with the Massachusetts Board of Registration in Nursing.
  • Previous home health or community-based nursing experience.
  • Strong organizational skills and the ability to manage a large caseload independently.
  • Bilingual in Spanish or Haitian Creole is a plus.
  • Comfortable with technology and proficient in Microsoft Word Excel and PowerPoint.
  • Demonstrated ability to coach and support staff and caregivers.
