Job Description
Location: Torrance CA
Duration: 13 weeks
Schedule: 8:00 AM 4:30 PM (Monday Friday EOW; 5x8s)
Pay Rate: $65/hr (W2) $69/hr (1099)
- Conduct biopsychosocial assessments and care coordination for patients across multiple units
- Develop and implement safe discharge plans including referrals to home health rehab or long-term care facilities
- Participate in treatment planning and facilitate family meetings
- Coordinate with community resources protective services and hospice/palliative teams
- Support patients and families facing issues related to mental health homelessness addiction or terminal illness
- Manage documentation in Epic according to hospital policies and discharge protocols
- Support and monitor up to 25 patients/day; cover up to 2 units on weekends
- Masters in Social Work (MSW) Required
- Minimum 2 years acute hospital case management experience Required
- Proficiency in Epic EHR Required
- Experience with:
- 51/50 holds POLST mental health crisis intervention
- AIDA PASRR adoption/foster care navigation
- Appeals and denials prior authorizations workers comp cases

- Knowledge of:
- Community resources Medicare/Medicaid advance directives protective services

- Emergency Department ICU L&D Medical-Surgical NICU Psych and Pediatrics units
