We are seeking a compassionate and experienced Nurse Practitioner to join our interdisciplinary palliative care team. You will provide high-quality home-based care across settings including private residences ALFs and SNFs. Responsibilities include managing chronic illnesses conducting comprehensive patient visits educating caregivers and collaborating with healthcare teams to deliver patient-centered care.

Role Highlights:
- Minimum 20 patient visits per week (typically 4 5/day)

Qualifications:
- Masters Degree in Nursing from an accredited program
- NP Certification (ANCC or AANP)
- Licensure or eligibility for licensure by the NC Board of Nursing
- Hospice/palliative certification preferred
