Sentara Home Care Services Virginia Beach is hiring a full time Care Transition Coordinator.
- Onsite at hospital M-F holiday rotation
Overview
Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.
Certification/Licensure
- Licensed RN LPN RT or BSW required. Proof of education as LPN RN RT or BSW required.
Experience
- 1 year of home care discharge planning or case management experience required.
