Job: Centralized Nurse - Care Coordinator Classification: Hourly/Non-Exempt Job Category: 2 Professional
Position Summary The Centralized Nurse - Care Coordinator provides patient services for patients who either need to be called who initiate a call or portal correspondence related to their health care needs. This included receiving a list of calls reviewing charts prioritizing call backs routing calls/requests to the appropriate staff refilling prescriptions and assisting with provider inboxes.
Essential Functions -Promptly returns calls and/or portal messages to patients and/or routes calls and messages to the appropriate clinic staff or physician to take appropriate action. -Offer evidence-based medical advice and instructions to patients regarding symptom management self-care and appropriate next steps for seeking further medical attention. -Accurately record patient assessments advice provided and any follow-up instructions in electronic medical records or other documentation systems. -Telephone duties involve triaging patients with acute medical needs via telephone conversation scheduling appropriate medical appointments for patients and communicating necessary information to providers and other medical offices. -Prioritizes callbacks based on need as determined by review of information given by patientphysician or others. -Reviews chart of patients requiring or requesting call back. -Determine the necessary follow-up for medical diagnoses and symptoms over the phone. -Refills prescriptions for patients and contact patient when refill as been sent to the pharmacy. -Creates and sends correspondence to patients per provider request regarding test results diet and lifestyle changes. -Oversees call center agents and assists with clinical questions that may present when patients call with varying levels of acuity and types of medical concerns. -Performs other related duties as assigned. -Adheres to all Titus Regional Medical Center (TRMC) and TCOT policies. -Follows and adheres to TRMC vaccine policy(s) mandated by the Centers of Medicare & Medicaid Services (CMS).
Skills -Excellent time management organizational and communication skills. -Must demonstrate consistent professional conduct and meticulous attention to detail. -Must possess excellent verbal and written communication skills as well as people skills with patients staff and other healthcare professionals. -Critical thinking skills willingness to learn and take initiative and positive outlook. -Must be able to deal with stress while maintaining composure. -Must have computer skills and dexterity required for data entry and retrieval of information. -Ability to provide direct patient care within the scope of ones nursing qualifications and licensure.
Work Experience -Prefer a minimum of three years recent clinical experience in a physician office home health critical care and/or emergency room. -Experience with EPIC electronic medical record (EMR) system and Microsoft Excel a plus. -Experience in telephone triage and Primary Care preferred
Education -Licensed Vocational Nurse Required or Registered Nurse Preferred -Bilingual in English and Spanish preferred
