Supports health programs such as chronic disease management program ensuring effective implementation and improving patient health outcomes. The role also involves coordinating Community Health Worker (CHW) training scheduling supporting their professional development conducting needs assessment developing programs and collaborating with healthcare providers and partners to improve patient outcomes.
This position will support our Merced and Stanislaus County Community Health Worker teams. Will work at our Modesto Paradise clinic.
Schedule is Monday Friday 8:00am 5:00pm.
Compensation: $26.87 - $28.22 an hour

Essential Duties and Responsibilities
- Supports the implementation of community health programs such as chronic disease management programs ensuring adherence to guidelines protocols and best practices.
- Assist with scheduling and overall logistical needs in the implementation of the various community health programs.
- Monitor program effectiveness track key performance indicators and prepare reports related to CHW activities and patient outcomes.
- Work collaboratively with healthcare providers partners and other stakeholders to ensure seamless care coordination and access to resources.
- Support CHWs in advocating for patients with chronic disease conditions ensuring their needs are met and barriers to care are addressed.
- Facilitate activities promoting health education and engagement with chronic disease programs.
- Ensure accurate and comprehensive documentation of CHW activities patient interactions and program outcomes.
- Identify areas for improvement in CHW training program implementation and patient support implementing changes as needed.
- Support CHWs in managing their caseloads prioritizing patients with complex needs and providing ongoing support and care coordination.
- Monitors the professional development of the clinical CHW to ensure that they have the knowledge skills and competence needed in providing education and support on specific chronic diseases such as diabetes hypertension and heart disease.
- Facilitate referrals to appropriate healthcare providers and community resources for patients with chronic diseases.

Physical Demands
The employee must occasionally lift and/or move up to 30 pounds. Must be able to hear patients on the phone and those who are served in-person and speak clearly in order to communicate information to patients and peers. Must be able to have vision that is adequate to read memos a computer screen and administrative documents. Must have high manual dexterity.
Work Environment
Exposure to biohazards including infectious material and waste and any other conditions common in a health care environment. Subject to unpleasant odors. The noise level in the work environment is usually quiet to moderate but may at times be noisy and crowded. Travel may be required at times.
Education/Experience Requirements
Minimum Qualifications:
- Excellent communication interpersonal and leadership skills.
- Ability to work independently and as part of a team.
- Knowledge of community health principles and resources.
- Understanding of the social determinants of health and their impact on chronic disease.
- Cultural competence and ability to work effectively with diverse populations.
- Strong computer skills and ability to use electronic health records systems.
- Strong knowledge of project coordination.
- Excellent time management and organizing skills
- Fluency in spoken and written English and other alternate language commonly spoken by GVHC patients. Pass alternate language assessment on an annual basis.
- Valid California drivers license vehicle insurance acceptable driving record and reliable transportation.
Education/Experience:
- High school diploma or equivalent.
- Bachelors degree in public health health science social work or related field preferred.
- Minimum two (2) years of experience in chronic disease management or health care.
