Job Description
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients with advancing age frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential. A minimum of two years of experience is required.

- Drivers license car insurance and personal vehicle for field work.
