As a Senior Care Coordinator at Horizon you will
- Provide independent person-centered care coordination for adults living with serious mental illness (SMI).
- Conduct comprehensive biopsychosocial assessments and develop individualized care plans that highlight each members needs goals and strengths.
- Manage an autonomous caseload of approximately 40 members ensuring high-quality timely and consistent care.
- Meet with highârisk members in their homes or community settings to provide intensive faceâtoâface support.
- Connect members with appropriate community resources benefits and services to enhance stability and wellbeing.
- Collaborate closely with interdisciplinary care teams to ensure coordinated effective service delivery.
- Employ creative problemâsolving to help members overcome practical barriers and achieve their care plan goals.
- Gain leadership experience by supervising and mentoring interns in the care coordination process.

What makes you a great candidate
We cant wait to learn more about you! Here are a few specifics of what youll need for the job:
- Bachelors Degree in a Human Services related field required.
- Five or more years of experience working with the mental health and substance use population required.
- A clean valid NYS drivers license three years of driving experience and reliable transportation to travel locally on a regular basis required.
- Leadership experience.
- Excellent verbal and written communication skills.
- Ability to work independently while also being part of a large team.
- Organizational time management and problem-solving skills.

Hours:
Monday through Friday - no weekend hours in this position! This position has a schedule from 8:00 AM - 5:00 PM on Monday Thursday and on Fridays 8:00am-2:00pm.
Compensation
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $23.41 - $27.00. Starting pay may be impacted based on experience and education. This position includes a bonus earning opportunity with an annual max potential of $4800 based on volume of service visits provided and cash equivalent opt out benefits. Compensation adjustments are evaluated annually.
Physical demands associated with this position include:
- Sitting for extended periods alternating between standing and sitting
- Local travel between worksites
- Mobility required involving repetitive wrist hand and finger movements.
- Handling light duties occasionally lifting objects up to 20 pounds
- Exposure to various environmental conditions including low/high temperatures outdoor elements.
