Job Description

Summary

Position Summary
Evaluates patients nutritional status and history. Plans recommends and implements nutrition care and education at National Jewish Health following the guidelines standards and procedures established by the department.

Essential Duties

  1. Performs nutritional assessment by using accurate and relevant data and information to identify nutrition-related problems.
  2. Identifies and implements appropriate purposely planned nutrition interventions as part of the patient care team. Prioritizes the nutrition diagnoses conferring with the patient and interdisciplinary team reviewing practice guidelines and policies.
  3. Provides nutritional counseling and education for patients and their families on normal and special nutrition.
  4. Initiates implements and/or modifies orders for diet or nutrition-related care. Manages nutrition support therapies. Perform nutrition related physicals.
  5. Documents diet information and recommendations in patient medical records per schedule and standards.
  6. Demonstrates competency in the use of the following department or institution software programs: Tempus Allscripts Affinity Patient Trak Microsoft Word and e-mail. Following key areas as outlined in corresponding competency modules.
  7. Performs special projects to support institution programs as requested. Develops and updates nutrition education materials teaching patient education classes QI participation projects presentations skills review participation.
  8. Participates in all required in-service team meetings and continuing education as scheduled. Ensures ongoing competency in the field by participating in relevant professional continuing education opportunities.
  9. Maintains a registered and active and CDR membership status as well as CDE status if applicable. Provides diabetes self-management education (DSME) and medical nutrition therapy to patients with diabetes and chronic kidney disease.

Addresses conflicts by focusing on the issues at hand to develop effective solutions when disputes or disagreements occur; helps others resolve conflicts by providing impartial mediation when needed.

Minimum Qualifications

  1. Education:Bachelors Degree in Food and Nutrition required. Masters Degree preferred.
  2. Work Experience:A minimum of 1 year of recent and related experience is required.
  3. Special Training Certification or Licensure:Registered Dietitian required. CDE preferred.

Salary Range: $30.29 - $40.06
