ABOUT THE PROGRAM: Sub-Acute Crisis Stabilization (SACS)
The Sub-Acute Crisis Stabilization (SACS) program provides short-term intensive behavioral health support in a residential setting to youth experiencing acute emotional or psychiatric crises. The program focuses on rapid de-escalation stabilization and connection to ongoing care through a collaborative trauma-informed approach. Services include comprehensive assessment individualized treatment planning therapeutic interventions and coordination with families schools and healthcare providers to ensure a safe and supported transition to the appropriate level of care.
POSITION SUMMARY: Child and Family APRN
The Child and Family APRN plays a critical role in the SACS program by delivering psychiatric evaluation medication management and clinical leadership in the stabilization and treatment of youth in crisis. Working as part of a multidisciplinary team the APRN provides direct care supports discharge planning and ensures continuity of services through collaboration with caregivers and community providers. This position is essential in promoting safety recovery and long-term wellness for youth and families navigating behavioral health challenges.
DUTIES & RESPONSIBILITIES:
Clinical Services
- Conduct comprehensive psychiatric evaluations including biopsychosocial assessments diagnostic formulation and treatment planning.
- Provide direct clinical care including medication management crisis intervention and brief therapy in alignment with agency standards and payer authorizations.
- Delivery of services such as history and physical exams follow-up care and referrals to medical specialists as needed.
- Assess level of care needs and coordinate transitions to appropriate services or levels of treatment.
- Maintain clinical documentation in accordance with agency policies including treatment plans progress notes and risk assessments.
Collaboration and Coordination
- Work closely with multidisciplinary teams including participation in case conferences team meetings and consultations.
- Coordinate care with external providers inpatient facilities and community health organizations to ensure continuity and quality of care.
Training and Leadership
- Provide clinical leadership and consultation to staff and teams as assigned.
- Offer training and psychoeducational interventions for clients and staff on topics such as wellness medication side effects and health education.
- Supervise staff when designated and contribute to the development of clinical programming.
Administrative Responsibilities
- Complete all required documentation for clinical services insurance authorizations and agency operations in a timely and accurate manner.
- Participate in agency and departmental meetings committees and quality improvement initiatives.
- Support agency operations by fulfilling additional duties as assigned by supervisors or program leadership.
Professional Development
- Engage in ongoing professional development through supervision training and continuing education.
QUALIFICATIONS:
Education: APRN
Experience: Experience in child medical services.
Licensure/Certification/Registration: CT APRN license.

COMPENSATION*:
$128960/yr minimum and up commensurate upon experience.
*Actual rates are determined at the time of offer and are based off of a strict calculation process as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note for part-time positions this range would be pro-rated based on hours worked)
