For more information go to: of Job The Work Unit: Residency Requirement: Class Code & Classification Description: MINIMUM QUALIFICATIONS: Six (6) years of professional experience as a RN public health nurse community health nurse healthcare clinic manager or another closely related field. OR A combination of related education and/or relevant experience in an occupation related to the work assigned equal to six (6) years. The position will be expected to work on site when needed at 4300 S Cherry Creek Dr Glendale CO 80246. Office hours are typically Monday through Friday from 8am to 5pm. Extended hours during the evening or the weekend MAY be required by business needs. A remote (home) office is required. Ability to travel independently or with a group throughout the state of Colorado for up to 16 hours/week. Some overnight travel may be required. All travel includes per diem and expenses will be covered as applicable. Overnight travel does not imply overnight work; rather hotel stays may be required depending on the event location. All job-related travel will be planned in advance in coordination with the position supervisor. Preferred and Essential Competencies: Relevant years of State service contribute to a candidates position-related knowledge skill ability behavior or other posted competency should be considered as a preferred qualification and given weight under the process. Previous experience working in a pediatric or family medicine clinical setting as a registered nurse or a medical practice manager knowledge of clinical protocols related to blood lead testing and follow-up and experience communicating complex health information to clinical and non-clinical audiences. Keen attention to detail with an ability to analyze facts data and evidence to arrive at a logical conclusion. Self-motivated with the ability to work well independently as well as in/with a team. The Colorado Childhood Lead Poisoning Prevention Program (CCLPPP) is under the direction of the Deputy Director and supports primary and secondary prevention strategies for childhood lead poisoning prevention and surveillance. The purpose of this program is to develop surveillance capacity to aid in preventing and ultimately eliminating childhood lead poisoning as a major public health problem in Colorado. The program encompasses a wide range of public health activities including case surveillance through laboratory test reporting data management and system improvement partnership and stakeholder engagement community care coordination enhanced analysis to identify at-risk geographies and populations and targeted outreach and prevention strategies for those at highest risk for lead poisoning. The program performs outreach and prevention strategies including in-home environmental investigations to identify sources of lead when test results indicate high blood lead levels (BLLs).
The program is responsible for ensuring blood lead testing and reporting enhancing blood lead surveillance improving linkages to recommended services and developing policies for targeted population-based interventions with a focus on community-based approaches for lead hazard elimination. CCLPPP responsibilities include sustaining a statewide Lead Advisory Committee maintaining a blood lead surveillance system that tracks all blood
lead test results and follow-up data on children with elevated blood lead levels including environmental source investigations and referrals to recommended services conducting analyses of surveillance data to identify lead-exposed children high-risk populations and geographic areas and partnering with programs and organizations
that provide services to mitigate the effects of elevated blood lead levels.
Summary of Position:
The purpose of this position is to serve as a nurse case manager within this capacity this position provides consultation and education to clinics and providers related to blood lead testing and reporting. Additionally this position provides caregivers with health education to reduce lead exposure and notifies clinicians of exposure sources found during investigations. This position also provides guidance to local public health agencies and clinicians related to reporting requirements and how data is used to provide follow-up services and educate others in the environmental investigation requirements and follow-up process. This position will provide blood lead testing at testing events around the state. This position applies quality control measures and evaluates data for compliance with regulations and requirements and engages reporters to achieve and maintain compliance.Minimum Qualifications Substitutions Conditions of Employment & Appeal Rights
