Job Summary

We are seeking a compassionate and organized Client Care Manager to join our team at 1776 Senior this role you will be the primary point of contact for families seeking in-home care services conducting initial assessments developing customized care plans and overseeing our team of caregivers to ensure high-quality service delivery. The ideal candidate is experienced in senior care has strong interpersonal skills and is passionate about supporting families through the caregiving process. This position requires a blend of administrative clinical and supervisory responsibilities to help our clients age comfortably and independently at home including oversight of in-home client care supervision of care staff ensuring compliance with IDPH and DOL regulations and occasional collaboration on marketing skilled nursing services.

Key Responsibilities

  • Client Intake and Assessments: Meet with families and potential clients to discuss their needs conduct in-home assessments and recommend appropriate care services. Build trusting relationships to guide them through the setup process. Serve as liaison between clients/client families and the office guide clients/client families when making decisions about care and document all observations and interactions.
  • Care Plan Development: Create personalized care plans based on client assessments incorporating medical emotional and daily living needs. Collaborate with families physicians and other healthcare professionals to ensure plans are comprehensive and effective. Provide oversight of client care through in-home visits and regular phone call check-ins.
  • Caregiver Supervision: Recruit train and supervise a team of caregivers including CNAs caregivers and nurses to ensure proper care. This includes scheduling performance evaluations and ongoing support. Monitor caregiver performance to maintain high standards of care and address any issues promptly. Communicate care plans to in-home staff provide guidance and on-the-job training as needed offer timely feedback (including disciplinary measures) and document all observations and interactions.
  • Quality Assurance: Regularly review and update care plans conduct follow-up visits with clients and families and ensure compliance with all relevant regulations and company policies. Fill in for occasional in-home skilled nursing needs as required.
  • Administrative Duties: Maintain accurate records of client interactions care plans and caregiver activities using our management software. Handle inquiries coordinate with external partners and contribute to team meetings.
  • Community Outreach: Assist in promoting 1776 Senior Care services through networking with local healthcare providers senior centers and community events. Work with the Director of Marketing to help formulate strategies for building skilled nursing services attend occasional marketing and networking events and visit long-term care facilities to strengthen relationships.

Qualifications

  • Education and Experience: Bachelors degree in Nursing Social Work Gerontology or a related field preferred. At least 2-3 years of experience in senior care home health or a similar role with proven experience in care planning and staff supervision.
  • Skills: Excellent communication and interpersonal skills for working with families clients and caregivers. Strong organizational abilities attention to detail and proficiency in Microsoft Office or similar tools. Knowledge of senior care regulations and best practices is essential including IDPH DOL and Joint Commission compliance.
  • Certifications: Valid drivers license and reliable transportation required (for in-home visits). CPR/First Aid certification preferred; RN or LPN license is a plus but not required.
  • Personal Attributes: Empathetic patient and adaptable with a genuine passion for senior care. Ability to handle sensitive situations with professionalism and discretion.
  • Availability: Must be available for flexible hours including some evenings or weekends as needed for client meetings or emergencies.
