Job Summary:
This role involves coordinating patient care within an acute care facility as a Licensed Master Social Worker (LMSW). The LMSW will collaborate with a unit of case managers floating between various units within a campus and potentially across campuses to ensure comprehensive care for patients in diverse settings such as the ED ICU and specialty units. A minimum of one year of experience is required.

Responsibilities:
- Collaborating with a team of case managers to coordinate patient care.
- Floating between different units within the primary campus and potentially to a secondary campus.
- Providing case management services to patients across various units (e.g. ED ICU specialty units).
- Coordinating care for patients in diverse settings and with varying needs.
- Performing other duties as assigned.

Required Skills & Certifications:
- Licensed Master Social Worker (LMSW).
- Minimum one year of experience in case management or a related field.
- Ability to work effectively in a fast-paced environment.
- Excellent communication and collaboration skills.
- Basic Life Support (BLS) certification.

Special Considerations:
- Weekend work is usually required every other weekend.
- Floating between units within the same campus is required; floating to a secondary campus is possible.
- All RTO restrictions must be provided at the time of submission.
- Business casual attire is required.
- EPIC charting system is used.
- Parking is provided by the facility.
