Job Summary:
This role involves providing skilled occupational therapy services to home health patients in Seaside OR and surrounding areas. The Occupational Therapist will assess patients needs develop and implement treatment plans and document progress using electronic health records. The position requires independent work managing a caseload of up to six patients daily and adhering to productivity standards.
Location: Seaside Oregon United States
Responsibilities:
- Conduct comprehensive occupational therapy evaluations to assess patients functional abilities and needs.
- Develop and implement individualized treatment plans based on assessment findings.
- Provide skilled occupational therapy interventions to improve patients functional performance and quality of life.
- Document patient progress and treatment plans accurately and timely in electronic health records (e.g. EPIC Oasis).
- Manage a caseload of up to six patients per day adhering to a 30-productivity point standard.
- Maintain professional communication with patients caregivers and other healthcare professionals.
- Comply with all applicable regulations and standards of practice.
- Manage travel between patient locations within a 60-mile radius.
Required Skills & Certifications:
- Current Occupational Therapy license in Oregon.
- Minimum of one year of skilled home health experience.
- Basic Life Support (BLS) certification.
- Proficiency in Oasis documentation.
- Valid drivers license car insurance and reliable transportation.
Preferred Skills & Certifications:
- Experience with electronic health record systems (EPIC).
Special Considerations:
- 8-hour day shifts; no 4x10 or 3x12 schedules available.
- No weekend work required.
- No on-call responsibilities.
- No floating between facilities.
- No block scheduling accommodations.
- Up to one shift per pay period may be called off by the facility.
- Travel within a 60-90 minute radius may be required.
- Business casual attire; scrubs allowed but not required.
