Job Summary:
The Territory Manager/Care Transition Coordinator is a hybrid role and will be directly responsible for sales and marketing activities and indirect patient care for a population of patients within a wide range of ages and home infusion therapy needs. The employee will participate in the promotion of specialty therapies and services targeting prescribers discharge planners nurses and other referral sources in the assigned area. Employee will work with physicians health systems pharma partners and other referral sources to support and understand their overall business objectives and critical success factors that drive patient discharges and prescriptions for infusion therapy services. The employee will function in an integral role to support the ongoing education of patients hospital staff physicians nursing agencies and community members regarding the programs and services of InfuCare Rx. The employee will identify new opportunities and participate in the development of new programs in order to promote InfuCare Rx services. The employee will be expected to work closely and collaborate with various other business units clinicians and staff members within the position requires the ability to work independently and with minimal supervision. Travel is required as part of the job. Fluent in both English and Spanish is required.
Primary Responsibilities - Territory Manager:
- Develops sales strategies to achieve company specific objectives and revenue goals.
- Interfaces with interdisciplinary team of clinicians and administrative support staff.
- Targets physician prescribers discharge planners and key decision makers and educates on companys patient specific services that are of value to referral sources and patients.
- Identifies and develops relationships with all sales targets that directly refer patients/prescriptions for specialty infusion medications.
- Familiarity with the geographical service area and territory assigned.
- Participates in community relations activities (i.e. health fairs exhibits educational programs in-services) as needed.
- Strategically/geographically plans field visits and customer educational opportunities to minimize travel expense and maximize productivity.
- Evaluates sales objectives periodically and adjusts marketing plan to achieve goals in planned timeframe.
- Collaborates with prescribers health systems and other partners to facilitate prescription/referrals.
- Identifies educational opportunities and provides presentations to referral sources.
- Identifies and develops patient care related solutions in partnership with internal team and customers.
- Performs other duties as required.
Primary Responsibilities - CTC:
- Coordination of referral for Home Infusion services by obtaining comprehensive referral information medical records review and verification of benefits to ensure eligibility of home infusion needs prior to hospital discharge.
- Confers with hospital discharge planner physicians home care agencies and other hospital staff as needed to educate and identify patient home infusion needs upon request by case manager social service and/or physician.
- Identifies patients home infusion needs through the review of medical record physician orders and interviews with patient and family.
- Identifies potential risk management problems for patients being discharged with home infusion services and reports cases to Director and/or Case Manager Supervisors and Company.
- Obtains essential information needed to determine an individuals eligibility for entry into home care. Entry is based on admission criteria; patient needs and companys ability to meet patient needs.
- Formulates a referral for home infusion services in collaboration with the prescribing physician professional staff and patient/family.
- Provides patients with information regarding home infusion services.
- Discusses medical insurance coverage related to home infusion services and patient financial responsibility with patient/family as necessary.
- Provides thecompanywith complete and accurate referral information prior to patient hospital discharge.
Responsible for understanding the expectations of the hospital and Company in the discharge planning process.
Discusses company/hospital service issues to formulate a plan for home infusion coordination and referral processes.
- Promotes a positive image for the hospital department and the home infusion company through collaboration and communication with hospital and home infusion company personnel.
- Responsible for participationin patient home infusion coordination of care processes.
- Works with home infusion company Pharmacy Nursing Intake and Quality Depts to ensure timely safe and accurate delivery and administration of medications supplies and equipment.
- Maintains start of infusion care as directed by physician.
- Identifies home infusion companys scope of services.
- Ensures customer satisfaction ratings are within hospital and home infusion company standards in case management measured areas.
- Complies with all HIPAA state federal regulatory and accrediting body requirements.
- Maintains current knowledge of insurance coverage payor contractual obligations and reimbursement regulations.
Physical Demands:
