As a Rehab Assistant you will assess and determine patient care needs and eligibility provide access and referrals to community services and engage with patients caregivers and other health care practitioners.

What will you do
Under the general direction of the Manager Patient Services and in accordance with OHAH Champlain standards legislation policies and guidelines the Rehabilitation Assistant is responsible for providing therapy assistance and administrative support to Physiotherapy and/or Occupational Therapy staff.
Primary Responsibilities:
Implements treatment plan with patient under the direction of a registered therapist
Assists registered therapists with patient assessments and treatment programs in accordance with the College of Physiotherapists of Ontario (CPO) and the College of Occupational Therapists of Ontario (COTO) guidelines
Completes and maintains documentation regarding patient intervention in accordance with OHAH Champlain policies and procedures and CPO/COTO guidelines
Meets with the registered therapist regularly to discuss patient treatment plan and progress
Researches assessment diagnosis and intervention tools/aids/materials to assist the registered therapist in delivering patient services
Carries out all duties in accordance with OHAH Champlain policies and procedures and Guidelines for the Use of Supportive Personnel developed by CPO and COTO
Maintains equipment and supplies on a regular basis by inspecting cleaning and sanitizing equipment used by clients and by scheduling equipment calibration/repairs with vendors
Installs assistive equipment and constructs therapeutic tools/devices for intervention as required by the therapist
Maintains organization of storage areas for therapy equipment and supplies to ensure that items are easily accessible
Monitors inventory levels and orders equipment and supplies as necessary to ensure a sufficient level of inventory is available
Prepares and organizes materials documents and activities for patient intervention as requested by the therapist
Assists therapist with documentation as required
Creates and maintains patient files in accordance with OHAH Champlain policies and procedures
Organizes files and maintains equipment catalogues and educational resources on a regular basis

Salary Range : $33.021/hr

What must you have
