Position Summary - Work collaboratively with clients community partners and other supportive services to identify effective solutions and transition clients from Central Connection to other service providers for ongoing care.
- Apply creative problem-solving skills to meet the specific needs of individuals who are involved with the Hamilton County Behavioral Health system.
What We Value in our next Transitional Case Manager - Bachelors degree in related mental health field
- Effective compassionate communication skills
- Ability to interact professionally with people from diverse backgrounds
- Knowledge of the Hamilton County public mental health system and other community resources
