Position Overview:
The HART Case Manager provides direct support to individuals experiencing acute emotional psychological or situational crises. The primary role is to assess client needs develop and implement crisis management plans and coordinate services that help stabilize individuals in crisis situations. This position requires strong communication crisis management skills and the ability to connect clients with appropriate resources for ongoing care and support.
Duties & Responsibilities:
Conduct comprehensive assessments of individuals in crisis to evaluate the severity of their situation and immediate needs.
Provide short-term intervention strategies including emotional support de-escalation and safety planning.
Develop individualized crisis intervention plans identifying immediate action steps and coordinating the appropriate resources.
Collaborate with crisis intervention teams law enforcement or medical professionals during high-risk or high-intensity situations.
Assist clients in understanding their situation and available options for treatment or stabilization.
Maintain accurate up-to-date case files and documentation ensuring compliance with confidentiality and legal requirements.
Document client interactions crisis interventions outcomes and referrals in accordance with organizational policies.
Connect clients to appropriate community resources including mental health services housing substance abuse programs healthcare and legal services.

Education:
Bachelors degree in social work Psychology Counseling or a related field is typically required.
Experience:
At least 2 years of experience in crisis intervention case management or social services
Current Texas Drivers License (upon hire)
Knowledge Skills and Abilities:
Proficient in MS Office Suite (Word Excel PowerPoint Outlook)
Excellent communication skills (verbal & written)
Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
Ability to maintain confidentiality when dealing with sensitive information
Ability to work well with public and staff members
Must use your personal mobile device for our Multifactor Authentication process
Must possess the ability to effectively present information in one-on-one and small group situations to other employees and/or managers
Must be knowledgeable of company protocols operational procedures and safety policies
Ability to de-escalate complex situations by using Trauma Informed Care and harm reduction practices

Position Type and Typical Hours of Work:
This is a full-time position.
Days and hours of work are typically Monday through Friday from 8am to 5 pm.
This position is required to be available for on-call duties as needed ensuring continuous support and coverage outside of regular business hours.
Other days and hours as necessary.
Work Environment:
This position involves both a professional office environment and fieldwork requiring travel to external locations for site assessments meetings and hands-on tasks.
Physical Demands:
Must be able to lift up to 50lbs have the ability to stand bend and or sit for extended periods of time.
