Position Overview:

The HART Case Manager provides direct support to individuals experiencing acute emotional psychological or situational crises. The primary role is to assess client needs develop and implement crisis management plans and coordinate services that help stabilize individuals in crisis situations. This position requires strong communication crisis management skills and the ability to connect clients with appropriate resources for ongoing care and support.

Duties & Responsibilities:

  • Conduct comprehensive assessments of individuals in crisis to evaluate the severity of their situation and immediate needs.

  • Provide short-term intervention strategies including emotional support de-escalation and safety planning.

  • Develop individualized crisis intervention plans identifying immediate action steps and coordinating the appropriate resources.

  • Collaborate with crisis intervention teams law enforcement or medical professionals during high-risk or high-intensity situations.

  • Assist clients in understanding their situation and available options for treatment or stabilization.

  • Maintain accurate up-to-date case files and documentation ensuring compliance with confidentiality and legal requirements.

  • Document client interactions crisis interventions outcomes and referrals in accordance with organizational policies.

  • Connect clients to appropriate community resources including mental health services housing substance abuse programs healthcare and legal services.

  • Follow up with clients to ensure that they are receiving the necessary services and make adjustments to intervention plans as needed.
