Description

JOB DESCRIPTION:

The Director of Health at Home provides strategic clinical and operational leadership over the delivery of care to patients in rural settings. This role ensures that compassionate patient-centered services are provided in accordance with federal state and Amberwell Health standards. The Director is responsible for overseeing clinical operations program development regulatory compliance community engagement staff development and financial sustainability of both Home Health and Hospice services. This position supports our mission to extend quality care to individuals and families where they need it mostat home.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide vision leadership and oversight for Amberwells Home Health & Hospice programs.
- Ensure regulatory and accreditation compliance with CMS KDHE and related bodies.
- Foster strong interdisciplinary teams and a culture of respect compassion and clinical excellence.
- Collaborate with physicians case managers discharge planners and referral partners to ensure appropriate care transitions.
- Direct quality improvement initiatives monitor performance metrics and lead patient satisfaction efforts.
- Develop and manage operational budgets control costs and pursue strategic growth aligned with the nonprofit mission.
- Engage with rural communities and faith-based groups to build referral relationships and community trust.
- Lead recruitment onboarding training and ongoing professional development of field and office staff.

Qualifications

JOB QUALIFICATIONS:

Experience:

  • Minimum Required Experience: 2 years of progressive leadership experience in home health and/or hospice including rural program oversight.
  • Preferred Experience: Experience with nonprofit healthcare value-based care models and interdisciplinary team management.

Education:

  • Minimum Required Education: Associate degree in Nursing
  • Preferred Education: Bachelors degree in nursing or higher education

Licenses:

  • Minimum Required Licenses: Active unencumbered KS or compact RN license unrestricted drivers license
  • Preferred Licenses:

Certifications:

  • Minimum Required: BLS
  • Preferred Certifications: Certified Hospice and Palliative Nurse or similar credential
