Job Summary:
This role is a community clinic-based case management position for a social worker. The case manager will conduct initial and 180-day assessments for assigned participants coordinate hospital and skilled nursing facility (SNF) discharges and collaborate with the interdisciplinary team (IDT) to ensure safe and appropriate discharge plans and continuity of care. Experience in case management and working within an IDT is required.

Responsibilities:
- Conduct initial and 180-day assessments for assigned participants.
- Coordinate hospital discharges collaborating with hospital discharge planners.
- Coordinate SNF discharges for participants requiring rehabilitation or skilled care.
- Collaborate with the interdisciplinary team (IDT) to ensure continuity of care.
- Assist in developing and implementing safe and appropriate discharge plans.
Required Skills & Certifications:
- Minimum one year of experience in case management.
- Current BLS certification.
- Master of Social Work (MSW) degree.
- One of the following active licenses: WA LICSW WA LSWAIC or WA LASW.
Preferred Skills & Certifications:
- Experience working within an interdisciplinary team (IDT) in a healthcare setting.
- Experience with PACE programs.
Special Considerations:
- This is a community clinic-based position.
- First-time travelers are not accepted; experience with travel assignments is preferred.
- No weekend or on-call requirements.
- RTO restrictions should be confirmed with the hiring manager.
Scheduling:
- Five 8-hour days per week.
- Specific schedule details to be determined.
