Community Support Team (CST) provides direct support to adults with a diagnosis of mental illness substance use or co- morbid disorder and who have complex and extensive treatment needs. This service consists of community-based mental health and substance use services and structured rehabilitative interventions intended to increase and restore an individuals ability to live successfully in the community.
The team approach involves structured face-to-face therapeutic interventions that assist in reestablishing the individuals community roles related to the following life domains: emotional behavioral social safety housing medical and health educational vocational and legal.
Job Duties include:
- Demonstrate use of appropriate knowledge skills abilities and judgement and provide therapeutic services that evidence quality performance and meet the needs of persons served
- Show knowledge and use of clinical process to achieve positive outcomes to include: evaluation assessment service treatment planning intervention techniques aftercare and transition planning
- Participate in community based team approach that utilizes the resources of the community consumer and treatment team to achieve positive outcomes
- Demonstrate the ability to manage an assigned caseload and meet the direct care billable productivity expectations of the organization
- Maintain accurate service and billing record documentation that clearly communicates the services provided and consumer progress
Education and Experience Required: A bachelors degree in Special Education Psychology Social Work or closely related Human Services area.
- For AP must have 1 year of professional work experience with population served
- For QP must have 2 years of professional work experience with population served.
- Exceptional written oral interpersonal and organizational skills.
- High level proficiency in Microsoft Office- MS Word Excel Access and PowerPoint. Or an equivalent combination of education and experience

We provide an array of services to Alexander Buncombe Burke Catawba Caldwell Iredell McDowell and Mitchell counties which consist of Assertive Community Treatment Team Community Support Team Day Treatment Integrated Care Intensive In-Home Medication Management Outpatient Therapy and Therapeutic Foster Care.
