Schedule: 40 hours per week 8a - 4:30pm with flexibility Monday - Friday

Work Location / Style: During the introductory period training will take place on-site at both our Holland and Muskegon locations. After training is complete the role will transition to a hybrid schedule with the expectation of working remotely three days per week and spending at least one day per week at each location (i.e. two days total on-site).

What Youll Do in This Role:

As an Enrollment Counselor youll guide potential participants and their families through the enrollment process ensuring a smooth and supportive experience from first contact to enrollment. Youll conduct eligibility assessments collaborate closely with the Interdisciplinary Team (IDT) and serve as a key liaison with referral sources and community partners. Your work will help increase awareness of LifeCircles PACE support outreach efforts and uphold compliance standards. This hybrid position supports both our Muskegon and Holland locations with scheduled on-site time required at each.


Primary Responsibilities:

  • Build strong collaborative relationships with teammates and communicate effectively across departments.
  • Adapt communication to meet the needs of older adults and individuals with varying cognitive abilities.
  • Conduct brief eligibility screenings over the phone or in person coordinate and schedule intake appointments.
  • Offer tours and host meetings with potential participants and their families at PACE centers.
  • Provide administrative support including data entry documentation records requests scheduling and general office tasks.
  • Accurately maintain information in CRM and EMR systems; assist with the deployment of documents and materials.
  • Assist participants and caregivers by gathering required documents for Medicaid applications; follow up with eligibility workers as needed.
  • Maintain regular outreach and follow-up with referral sources caregivers and community partners.
  • Complete 2- and 6-month follow-up calls with new enrollees collaborating with the care team to address concerns.
  • Support community outreach initiatives and help coordinate events and awareness activities.
  • Contribute to quality improvement efforts through reporting documentation and participation in QAPI processes.

What Youll Need:

  • High school diploma required; associate or bachelors degree preferred.
  • Minimum of 1 year of experience in long-term care healthcare or a related field.
  • Exceptional customer service and communication skills including professional phone etiquette.
  • Familiarity with the challenges of aging caregiver stress and the needs of older adults.
  • Bilingual in English and Spanish required.
