Job Description
BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors we feed shelter teach and support our neighbors to build a stronger community. QQUALIFICATIONS
- Bachelors degree.
- Proficiency in English required; proficiency in a second language preferred.
- Proficiency in modern business communications including personal computers electronic mail voicemail facsimile and copier equipment.
ESSENTIAL DUTIES AND RESPONSABILITIES
- Provide one on one case management services to building/neighborhood residents.
- If applicable conduct site visits to rapid rehousing DV clients
- Participate in all mandatory agency trainings.
- Attend internal and external agency/contract meetings.
- Participate in monthly staff meetings or more frequently as required.
- Meet monthly with Program Director/Assistant Program Director for exchange of ideas and written feedback.
- Submit monthly reports to supervisor.
- Perform additional duties as assigned
PROGRAM SPECIFIC RESPONSABILITIES
- The case manager will carry a case load
- S/He will direct the client to the appropriate service or program and will complete a prescreening/intake.
- The case manager will enter information into Salesforce databases confirming whether the referral was accepted
- The case manager will enter updated information into Salesforce database concerning missed appointments declined services etc.
- The case manager will continue to enter information regarding the status of the clients case.
- The case manager will attend meetings and conduct presentations
- The case manager will assist the Program Director writing reports to funders
- The case manager will conduct ongoing monthly or bi-monthly home visits with staff
- All other duties as requested by the supervisor
PHYSICAL REQUIREMENTS
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and/or move up to 10 pounds
- Ability to stand walk or sit for long periods of time
- Ability to climb five flights of stairs if required to conduct home visits and/or fieldwork
- Ability to bend and retrieve objects and/or documents
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work
